1. Thick walls and floors
Concrete, reinforced concrete and thick partitions can strongly attenuate the signal. Distance matters, but so does the entire path between the router and the device.
2. Poor router placement
A router in a corner, on the floor or inside a closed cabinet can provide much worse coverage than the same router in a more central, open position.
3. Too many obstacles
Several walls along the path can be a bigger problem than one wall farther away. This is why the floor plan matters, not just the number of metres.
4. Radio interference
Neighbouring WiFi networks and other sources of interference can reduce connection quality. In an apartment building, this can be a much bigger issue than in a detached house.
5. A poorly placed mesh node
A mesh node should not normally be placed deep inside the dead zone. It needs to sit between the router and the weak area so that it can maintain a good connection back to the router.
